Zusammenfassung

Reserve Care (formerly Hospice of the Western Reserve) is seeking a State-tested Nursing Assistant to provide personal care under nurse supervision in a home-care setting. You will work with a transdisciplinary team to support patients’ ADLs, comfort and safety, and participate in care conferences and ongoing education.

Responsibilities include charting, records, and ensuring accurate observation notes during visits, with flexibility for variable shifts and weekends as needed.

Qualifikationen

• Must be able to read and write English proficiently.
• Must be state tested nursing assistant in good standing.
• Equivalent education/experience may substitute for minimum qualifications.

Aufgaben

• Provide personal care under supervision of a nurse according to the patient’s plan of care.
• Collaborate with a transdisciplinary team in various settings.
• Assist with ADLs and safe environment for patients.
• Record activities and observations in the clinical record during visits.
• Attend patient care conferences and team meetings as appropriate.
• Maintain charting, timesheets and mileage logs per agency policy.
