Job summary

Luminary Hospice in Indiana seeks a compassionate hospice aide to support patients under RN supervision. You will provide personal care, monitor patient conditions, document observations, and ensure comfort and dignity in end-of-life care.

Responsibilities include assisting with daily living activities, coordinating with the Case Manager, following safety protocols, and maintaining accurate records. Certification and a valid driver’s license are required.

Qualifications

• Active STNA or CNA certification in the state where duties are performed.
• Must be at least 18 years old.
• Ability to read and follow written instructions and document care.
• Current CPR certification.
• Satisfactory references from previous employers.
• Licensed driver with an automobile that is insured in accordance with organization requirements and is in good working order.
• Willingness to work in patients' homes and provide compassionate end-of-life care.

Responsibilities

• Perform personal care and assist with daily living activities.
• Observe patient conditions and report findings to the Case Manager.
• Provide safe transfers and mobility assistance.
• Maintain a clean living area and proper hygiene.
• Communicate with patients and families with empathy and respect.
• Adhere to care procedures, documentation, and professional standards.
