Job summary

Luminary Hospice in South Bend, Indiana, is seeking a compassionate Hospice Aide to support patients and families in their end-of-life journey under RN supervision. You will perform personal care, observe patient needs, and document care to promote comfort and safety.

Requirements include active STNA or CNA certification, completion of required training hours, CPR certification, and a driver’s license. Join a mission-driven team dedicated to dignity, empathy, and quality care.

Qualifications

• Active STNA or CNA certification required.
• Meets state training requirements.
• Minimum 75 hours classroom and supervised training; 16 hours practical.
• Must be at least 18 years old.
• Able to read and follow written instructions and document care.
• Understands hospice philosophy and terminally ill care.
• Satisfactory references from previous employers.
• Self-directed with minimal supervision and ability to cope with death/dying.
• Empathy for ill and impaired patients.
• CPR certification current.
• Licensed driver with insured vehicle in good condition.
• Complete a probationary period with supervision.

Responsibilities

• Perform personal care tasks and respond promptly to patient requests.
• Communicate with patient and family with confidentiality and respect.
• Ensure safety and proper use of equipment (rails, O2, etc.).
• Assist with baths, oral hygiene, grooming, and changing bedding.
• Assist with dressing and undressing patients.
• Plan and prepare nutritious meals as assigned and assist with feeding.
• Record vital signs as ordered and report changes to Case Manager.
• Monitor skin condition to prevent breakdown over bony prominences.
• Observe and report patient condition and significant changes to Case Manager.
• Assist with ambulation and transfers as directed by staff.
