Job summary

Luminary Hospice is seeking a dedicated hospice aide to support patients at home under RN supervision. Responsibilities include personal care, safety, meals, and daily monitoring to promote comfort and dignity at end of life.

The role requires STNA/CNA certification, 75 hours of training, 18+ years old, and CPR certification. Excellent communication and empathy are essential for working with patients and families.

Qualifications

• Requires STNA/CNA certification valid in the state of duties.
• Minimum 75 hours of classroom and supervised training (16 hours practical).
• Must be 18+ years old and able to read and follow written instructions.
• CPR certification required.

Responsibilities

• Provide personal care and attend to patient requests promptly.
• Communicate with patient and family, maintaining confidentiality.
• Ensure patient safety and proper use of equipment.
• Assist with baths, grooming, oral hygiene, and changing linens.
• Assist with dressing and meals as assigned.
• Take and record vital signs as ordered.
• Observe skin condition to prevent tissue breakdown.
• Report changes to the Case Manager.
• Assist in ambulation and transfers as directed.
• Reminders for medication per state regulations.
• Keep living area clean and orderly.
• Adhere to documentation and care procedures.
• Participate in quality improvement activities.
• Respect patient and caregiver environment.
