Job summary

Luminary Hospice is seeking a dedicated hospice aide to join our interdisciplinary team in a patient home setting. You will work under the supervision of a registered nurse to observe, document, and deliver compassionate personal care to patients at end of life.

Qualifications

• Active STNA or CNA certification as required by state.
• Meets the training requirements of the State.
• Completed minimum of 75 hours of classroom and supervised practical training.
• At least 18 years of age.
• Ability to read and follow written instructions.
• Understands hospice philosophy and providing specialized care to the terminally ill.
• Satisfactory references from previous employers.
• Self-directed with the ability to work with little supervision and maintain composure.
• Empathy for the needs of the ill and impaired.
• Possess and maintain CPR certification.
• Tact, patience, and good personal hygiene.
• Licensed driver with insured vehicle.
• Complete a probationary period with supervisor documentation.

Responsibilities

• Perform personal care tasks after patient assignment and respond promptly to requests.
• Communicate with patients and families in a respectful, confidential manner.
• Ensure patient safety and use equipment properly (e.g., rails, oxygen).
• Provide personal care including baths, grooming, and changing bed linens.
• Assist in dressing and undressing the patient as assigned.
• Plan and prepare nutritious meals and assist with feeding as needed.
• Record vital signs and report changes to the Case Manager.
• Maintain patient skin care and prevent tissue breakdown.
• Observe and report changes in patient condition to the Case Manager.
• Assist with ambulation and transfers as directed by the nurse.
• Support in range of motion and simple therapeutic activities.
• Assist with medications reminders as allowed by state regulations.
• Offer bedpans and urinals and light laundry as needed.
• Maintain a clean and orderly living area for the patient.
• Adhere to documentation and care procedures and professional conduct.
• Participate in quality improvement activities.
• Respect patient and family privacy and preferences.
• May perform additional tasks as assigned.
