Job summary

Apploi is seeking a compassionate hospice aide to support patients under the supervision of a registered nurse, carrying out the plan of care to promote comfort and meet personal needs. The aide will report observations and care performed to ensure quality and safety in patient care.

The role includes assisting with daily living activities, providing respectful communication to patients and families, and helping with safety and comfort measures, while adhering to care standards and documentation

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC) credential.
• Current Healthcare Provider CPR certification (hands-on).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and comfort with terminal care.
• 1 year hospice/home health experience preferred.
• Basic computer skills to document in EMR.

Responsibilities

• Perform personal care and attend to patient requests promptly.
• Communicate with patient and family respectfully and confidentially.
• Ensure safety and proper use of equipment.
• Provide baths, skincare, oral hygiene, and bed linen changes.
• Assist with dressing, undressing, and feeding as assigned.
• Support range of motion, transfers, and ambulation per plan.
• Report changes to RN Case Manager and document observations.
• Assist with medications reminders as allowed by state rules.
• Keep patient area clean and orderly and maintain documentation.
