Job summary

Qualifications

• High school diploma preferred; multiple approved training/competency paths exist.
• Experience as Hospice Aide or Nursing Assistant preferred.
• Ability to read, write and report clinical information clearly.

Responsibilities

• Provide hands-on personal care per each patient’s plan of care.
• Assist transfers and walking to move patients safely.
• Support nutrition and hydration for patients.
• Keep the home clean, safe and comfortable with infection control.
• Notice and report changes in patient condition and document care.
• Communicate with kindness and respect to patients and families.
• Respect privacy, dignity and the patient’s home.
• Share the hospice philosophy of compassionate, high-quality care.
