Qualifications

• Education: High School Diploma preferred; programs or competency evaluation as per Medicare regulations.
• Experience: At least one year as Hospice Aide or Nursing Assistant in healthcare settings preferred.
• Skills: Read, write and verbally report clinical information to patients, families, and staff.
• Transportation: Reliable transportation, valid license, and auto liability insurance.

Responsibilities

• Provide direct patient care under the direction of the RN and according to the Aide Plan of Care.
• Assist with safe transfers and ambulation per Hospice policy.
• Report changes and document care to ensure continuity of care.
• Maintain infection control and provide a clean, safe home environment.
• Communicate respectfully with patients, families, and staff.
• Respect patient privacy and property.
• Assist with proper nutrition and fluid intake per plan.
• Participate in hospice-sponsored in-service programs and maintenance of competencies.
• Perform on-call/after-hours duties as assigned and ensure timely paperwork.
