Qualifications

• Reading, writing and reporting clinical information clearly.
• Strong interpersonal communication with patients, families and staff.
• Ability to travel to patient homes and work locally.

Responsibilities

• Provide direct patient care under RN supervision per Plan of Care.
• Assist with safe transfers and ambulation.
• Report changes and document care to ensure continuity.
• Maintain infection control and a safe home environment.
• Communicate respectfully with patients, families and staff.
• Complete required paperwork and meet productivity standards.
