Qualifications

• Education: High School Diploma preferred with listed training paths.
• Experience: at least one year as Hospice Aide or CNA preferred.
• Skills: able to read, write and report clinical information clearly.
• Transportation: reliable transportation and auto insurance.

Responsibilities

• Provide direct patient care under RN direction per Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care to ensure continuity.
• Maintain infection control and a clean, safe environment.
• Promote respectful communication with patients, families and staff.
• Assist with proper nutrition and fluid intake.
• Participate in hospice-sponsored training and maintain competency.
• On-call/after-hours duties including weekends/holidays.
• Submit required paperwork in a timely manner.
• Meet productivity standards.
