Job summary

Heart’n Soul Hospice is seeking a dedicated Hospice Aide to provide direct patient care under the supervision of an RN. You will assist with safe transfers, report changes, and document care to ensure continuity of patient services.

Ideal candidates hold an unencumbered CNA certification, a valid CNA license, and at least one year of hospice or nursing assistant experience in healthcare settings. A valid driver's license and reliable transportation are required.

Qualifications

• Unencumbered certification according to the Nurse Aide Registry.
• Current CNA license.
• At least one year experience as Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long-term care facility.
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care.
• High integrity, including maintenance of confidential information.
• Must have current driver's license, reliable transportation, and ongoing valid auto liability insurance.

Responsibilities

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy.
• Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
