Job summary

Heart to Heart Hospice seeks a compassionate Hospice Aide (CNA) to join our care team. Under the supervision of an RN, you’ll provide direct patient care, assist with transfers, and document care to ensure continuity and quality of life for patients with life-limiting illnesses.

We value integrity, confidentiality, and a commitment to exceptional bedside manners. A valid CNA license, current driver’s license, and reliable transportation are required.

Qualifications

• High School Diploma preferred or GED.
• Unencumbered certification according to Nurse Aide Registry.
• Current CNA license.
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility.
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care.
• High integrity, including maintenance of confidential information.
• Current driver's license, reliable transportation, and ongoing auto liability insurance.

Responsibilities

• Provide direct patient care to patients under the RN's direction per the Hospice Aide/Homemaker Plan of Care.
• Assist patients with safe transfers and ambulation per agency policy.
• Report and document pertinent information to ensure continuity of care.
• Practice infection control principles.
• Promote positive, supportive, respectful communication to patient/family and staff.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and personnel policies.

What You Must Have:

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/HomemakerPlan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicatechanges and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
