Job summary

Heart to Heart Hospice is seeking a compassionate Hospice Aide (CNA) to help provide direct patient care under the supervision of the RN, supporting transfers and daily living activities in accordance with the Plan of Care.

The role requires a current CNA license, reliable transportation, and a willingness to work as part of a dedicated care team in a hospice setting. This is a full-time opportunity with benefits and growth potential.

Qualifications

• High School Diploma preferred or GED.
• Unencumbered CNA license.
• At least one year experience as Hospice Aide or Nursing Assistant.
• Interest in welfare of ill and elderly; compassionate care.
• High integrity and confidentiality.
• EMR experience a plus.
• Driver's license and reliable transportation with auto insurance.

Responsibilities

• Provide direct patient care under RN and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report and document care to ensure continuity.
• Practice infection control principles.
• Promote respectful communication with patient/family and staff.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with laws and Medicare regulations.

Hospice Aide - CNA

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/HomemakerPlan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicatechanges and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
