Job summary

Heart to Heart Hospice in Sherman, TX is seeking a dedicated Hospice Aide (CNA) to provide direct patient care under RN supervision, assisting with transfers, hygiene, and daily living activities per the Plan of Care.

You will report changes, maintain infection control, and communicate with patients, families, and team members, ensuring compassionate, compliant care while documenting all services and complying with regulations.

Qualifications

• High School Diploma or GED preferred.
• Current CNA license required.
• At least one year experience as Hospice Aide or Nursing Assistant in relevant settings.
• Demonstrates interest in welfare of ill and elderly; compassionate care.
• EMR experience is a plus.
• Must have valid driver's license and transportation.

Responsibilities

• Provide direct patient care under RN supervision and Plan of Care.
• Assist patient with safe transfers and ambulation as per policy.
• Report/communicate changes and document care provided to ensure continuity of care.
• Practice infection control principles.
• Promote respectful communication with patient, family and staff.
• Submit documentation promptly and accurately.
• Perform other duties as required.
• Ensure compliance with laws, regulations, and policies.

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/HomemakerPlan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicatechanges and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
