Job summary

Heart to Heart Hospice in Humble, TX seeks a dedicated Hospice Aide (CNA) to join our patient-centered care team. Under RN supervision, you will provide direct patient care and assist with transfers, ambulation, and documentation to ensure continuity of care.

You will adhere to infection control standards, communicate with patients and families compassionately, submit timely records, and support compliance with healthcare regulations and facility policies.

Qualifications

• High School Diploma or GED preferred.
• Current CNA license and unencumbered Nurse Aide Registry.
• At least one year experience as Hospice Aide or Nursing Assistant.
• EMR experience is a plus.

Responsibilities

• Provide direct patient care to patients under the RN supervision and according to the Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report/communicate changes and document care to ensure continuity of care.
• Practice infection control principles.
• Promote respectful communication with patient/family and staff.
• Timely submission of all documentation and duties as required.
• Ensure compliance with laws, Medicare regulations, and policies.

Hospice Aide - CNA

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/HomemakerPlan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicatechanges and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
