Job summary

Heart to Heart Hospice is seeking a compassionate Hospice Aide (CNA) to provide direct patient care under RN supervision. You will assist with transfers and ambulation, document care, and communicate changes to ensure quality patient outcomes.

The role requires an unencumbered CNA license, high school diploma or GED, and at least 1 year of hospice or nursing assistant experience. A valid driver’s license and reliable transportation are also needed.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA certification.
• Current CNA license.
• At least one year experience as a Hospice Aide or Nursing Assistant.
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care.
• High integrity, including maintenance of confidential information.
• Must have current driver's license, reliable transportation, and provide ongoing valid auto liability insurance.

Responsibilities

• Provide direct patient care under the RN and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care provided to ensure continuity of care.
• Practice infection control principles.
• Promote positive, respectful communication with patient/family and staff.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations, and policies.

What You Must Have:

• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/HomemakerPlan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicatechanges and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
