Job summary

Heart to Heart Hospice in Mishawaka, IN is seeking a dedicated Hospice Aide (CNA) to provide compassionate direct patient care under RN supervision, supporting patients with life-limiting illnesses and their families in home or facility settings.

The role requires an unencumbered CNA license, High School Diploma or GED, at least one year of hospice or nursing assistant experience, valid driver’s license, and reliable transportation.

Qualifications

• High School Diploma preferred or GED.
• Unencumbered CNA license required.
• Minimum of one year experience as Hospice Aide or Nursing Assistant.
• Demonstrates genuine care for ill and elderly; maintains confidentiality.
• Must have current driver’s license, reliable transportation, and valid auto liability insurance.

Responsibilities

• Provide direct patient care under the RN direction and Plan of Care.
• Assist patients with safe transfers and ambulation per policy.
• Report and document changes and care provided to ensure continuity of care.
• Follow infection control principles.
• Maintain respectful communication with patient/family and staff.
• Submit all documentation in a timely manner; comply with laws and policies.

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/HomemakerPlan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicatechanges and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
