Job summary

St. Croix Hospice in Seymour, IN is seeking a compassionate Hospice Aide to join our interdisciplinary care team under RN supervision. You will provide direct patient care, assist with bathing, ADLs, and personal hygiene, while maintaining comfort and safety.

You will take and record vital signs, assist with medications per plan, plan nutritious meals, and report changes to the RN Case Manager. This role offers 1-1 patient engagement and opportunities for growth within our local and regional

Qualifications

• At least 18 years of age.
• Active Certified Nursing Assistant (CNA) designation current with state-specific requirements.
• Licensed and insured driver with automobile in good working order.

Responsibilities

• Provide patient care including bathing, nail/skin care, dressing/undressing, dental and personal hygiene.
• Take and record vital signs and assist with self-administered medications.
• Plan and prepare nutritious meals and assist with feeding as needed.
• Assess and report patient condition and changes to the RN Case Manager.
• Assist in ambulation, transfers, positioning and therapy activities.
