Job summary

St. Croix Hospice in Newburgh, IN seeks an Entry Hospice Aide to join our patient-focused family. Under RN supervision, you will provide quality, safe care, assist with activities of daily living, and ensure timely, accurate documentation of care for each patient.

You will perform bathing, dressing, transfers, vital signs, meal support, and report changes to the RN Case Manager. The role requires empathy, tact, and a commitment to HIPAA privacy standards while delivering compassionate, 1-on-1

Qualifications

• Active CNA certification current with state requirements.
• Be at least 18 years old and eligible to drive.
• Ability to follow RN guidance and document care accurately.

Responsibilities

• Provide patient care including bathing, dressing, and personal hygiene.
• Record vital signs and assist with medications under supervision.
• Plan meals and assist with feeding as needed.
• Report patient condition changes to the RN Case Manager.
• Assist with mobility, transfers and range of motion therapies.

As the Hospice Aide, you are a paraprofessional and valued member of the interdisciplinary care team. You will work under RN supervision and focus on quality, continuity, and safe patient care. In this role you will be responsible for timely and accurate documentation of care.

JOB RESPONSIBILITIES

• You will provide patient care including bathing, nail/skin care, dressing/undressing, dental and other personal hygiene while maintaining a sanitary and orderly environment
• You will offer positive, respectful, and confidential patient/family communication
• You will be responsible for taking and recording vital signs, and assisting with self-administered medications
• You will plan and prepare nutritious meals, including shopping and feeding the patient as needed
• You will assess and report patient's condition and changes to the RN Case Manager
• You will assist in ambulation, exercise, transfers, positioning and support a complete range of motion and other therapy activities
