Job summary

St. Croix Hospice is hiring a Hospice Aide to support our interdisciplinary care team under RN supervision. You will provide direct patient care, document accurately, and help maintain safety and quality in daily routines.

You will collaborate with nurses and therapists to monitor patients, assist with activities of daily living, and support families with compassionate care. This role emphasizes dignity, comfort, and timely reporting of changes.

Qualifications

• Excellent interpersonal skills and compassionate patient care.

• Strong sense of privacy and HIPAA compliance.

Responsibilities

• Provide patient care including bathing, nail/skin care, dressing and undressing.
• Communicate with patients and families in a positive, confidential manner.
• Record vital signs and assist with self-administered medications.
• Plan and prepare nutritious meals and assist with feeding as needed.
• Assess patient condition and report changes to RN Case Manager.
• Assist with ambulation, transfers, positioning and therapeutic activities.

Job Responsibilities

• You will provide patient care including bathing, nail/skin care, dressing/undressing, dental and other personal hygiene while maintaining a sanitary and orderly environment
• You will offer positive, respectful, and confidential patient/family communication
• You will be responsible for taking and recording vital signs, and assisting with self-administered medications
• You will plan and prepare nutritious meals, including shopping and feeding the patient as needed
• You will assess and report patient\'s condition and changes to the RN Case Manager
• You will assist in ambulation, exercise, transfers, positioning and support a complete range of motion and other therapy activities

Qualifications

• At least 18 years of age
