Job summary

Heart to Heart Hospice is seeking a compassionate Hospice Aide / CNA to deliver direct patient care under the supervision of an RN, following the Plan of Care.

Responsibilities include safe transfers, report changes, documentation for continuity of care, infection control, respectful communication with patients and families, and timely completion of records. A valid CNA license and high school diploma are required, with EMR experience a plus.

Qualifications

• High School Diploma or GED preferred.
• Unencumbered CNA license required.
• Minimum one year of hospice or nursing assistant experience preferred.
• Demonstrates compassionate care for ill and elderly.

Responsibilities

• Provide direct patient care under RN supervision and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care to ensure continuity of care.
• Practice infection control principles.
• Communicate respectfully with patient, family, and team.
• Submit documentation timely.
• Perform other duties as required.

Hospice Aide - CNA

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
