Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to provide direct patient care under the supervision of a registered nurse. You will assist with transfers, mobility, and documentation while upholding infection control and compassionate communication with patients and families.

The role requires a current CNA license, at least one year of hospice or nursing assistant experience, and a valid driver’s license.

Qualifications

• Unencumbered CNA license current on Nurse Aide Registry.
• High School Diploma or GED preferred.
• At least 1 year Hospice Aide or Nursing Assistant experience in healthcare.
• Valid driver's license and reliable transportation.
• Ability to maintain confidentiality and provide compassionate care.
• EMR experience is a plus.
• Interest in welfare of ill and elderly and compassionate care.

Responsibilities

• Provide direct patient care under RN direction and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report and document changes and care provided for continuity of care.
• Practice infection control principles.
• Communicate respectfully with patient, family, and staff.
• Submit all documentation timely.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws and Medicare regulations.

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/HomemakerPlan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicatechanges and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
