St. Croix Hospice in Grand Rapids, MN is hiring a Hospice Aide (CNA) to support patients at home under RN supervision. You will provide direct personal care, monitor conditions, and help maintain comfort and dignity.

Candidates must hold an active Minnesota CNA license, be 18+, have a driver's license, pass background check, and be comfortable in a home setting. We offer competitive pay and benefits.

Competenze

• Must hold an active CNA license in the state of practice (Minnesota).
• Ability to provide direct patient care and personal support.
• Experience with home health care and end-of-life care is a plus.

Mansioni

• Provide personal care including bathing, dressing, oral hygiene, and bed linens changes.
• Assist with mobility, transfers, and instructed exercises.
• Support meals, feeding, and light housekeeping tasks.
• Monitor patient condition changes and report to RN Case Manager.
• Maintain documentation and ensure HIPAA compliance.
• Provide emotional support to patients and respite for families.
