Job summary

St. Croix Hospice seeks a compassionate Hospice Aide to deliver hands-on personal care and comfort in patients' homes as part of an interdisciplinary team.

You will follow the Plan of Care, provide in-person care, and assist in daily activities to support quality of life. The role requires an active CNA license and ability to meet state requirements, with duties including documentation, coordination with the RN Case Manager, and maintaining a safe home environment to ease patient and family

Qualifications

• Must have an active CNA license in the state of practice.
• Must be at least 18 years old.
• Ability to pass background study, consumer report and drug test.
• Valid driver's license and access to a reliable vehicle.

Responsibilities

• Provide direct personal care and support for hospice patients.
• Assist with daily activities, meals, hygiene.
• Monitor patient condition and report changes to RN Case Manager.
• Maintain documentation and coordinate care within care plan.
• Ensure a safe, clean home environment and patient comfort.
• Offer emotional support to patients and families.
