Job summary

AccentCare is seeking an After Hours Admission RN to join our hospice team serving Bucks, Montgomery, Philadelphia, Delaware, and Chester Counties. This full-time role offers a 5pm–8am schedule with a 7 on/7 off rotation and compensation in the $90k–$95k range based on experience.

The role requires current RN licensure, prior hospice/home health or similar experience, and strong communication and organizational skills.

Qualifications

• RN licensure in the applicable state, associate degree preferred.
• Must participate in on-call rotation and have reliable transportation.
• Strong communication and organizational skills; proficient in MS Office and EMR.

Responsibilities

• Conduct comprehensive nursing assessments with the IDG to develop, implement and evaluate care plans.
• Maintain timely documentation and coordinate care resources to support complex cases.
• Educate patients and caregivers and support admissions and care transitions.
• Coordinate care as Care Manager or in an on-call capacity; participate in IDG meetings and family conferences.
