Job summary

UnitedHealth Group, in partnership with Geisinger Hospice-Selinsgrove, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care for patients and families after regular hours, including nights and weekends.

The RN will triage needs by phone, perform initial evaluations for hospice eligibility within 48 hours, and conduct afterhours visits—admissions, recertifications, and routine care—while ensuring compassionate, patient-centered support.

Qualifications

• Must have unrestricted RN licensure in the state of practice.
• Valid driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Triage patient needs by phone until a nurse can provide an in-person visit.
• Respond to crisis calls from patients and families afterhours with urgency.
• Perform the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Conduct afterhours visits including admissions, death, recertification, and routine visits.
• Identify and reassess patient/family needs at least every 15 days.
• Document problems, goals, interventions, and patient/family responses to hospice care.
• Provide care in diverse home situations regardless of age, race, creed, color, sex, disability, or financial condition.
