Job summary

Residential Home Health and Hospice is hiring an After Hours Registered Nurse (RN) to provide end-of-life care in patients' homes in the Troy area. This role offers a $10,000 first-year bonus upon achieving milestones.

You will respond to after-hours needs, manage symptoms, and support families during evenings, nights, weekends, and crises. Travel within the territory is required and EMR documentation must be accurate and timely.

Qualifications

• Active RN license in the state you work.
• Two years RN experience and hospice care preferred.
• Valid driver’s license and ability to drive approximately 20% of the day to patient locations.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes.
• Respond to after-hours and on-call needs with urgent assessments and symptom management.
• Support and guide patients and families during evenings, nights, weekends, and crises.
• Coordinate care with the interdisciplinary team and document in EMR accurately and timely.
• Work independently, but travel to patient homes within the assigned territory.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life


• Respond to after-hours and on-call patient needs by performing urgent assessments, managing symptoms and pain, and providing compassionate end-of-life support in real time


• Support and guide patients and families during evenings, nights, weekends, and crisis situations, ensuring they feel informed, reassured, and cared for throughout the hospice journey


• Apply strong clinical judgment and critical thinking to coordinate care needs, respond to changing patient conditions, and maintain accurate, timely, and compliant EMR documentation


• Work independently, but never alone

After Hours RN Qualifications

• Active Registered Nurse, RN license in the state you work


• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred


• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
