AHN Healthcare@Home is seeking a Weekend Registered Nurse (RN) to provide end-of-life hospice care to patients in their homes in the Erie, PA area. A $10,000 first-year success bonus is available for achieving key milestones during the first year.

The role requires an active RN license in the state of work, at least two years of RN experience (hospice experience preferred), and a valid driver's license with up to 20% travel to patient locations.

Responsibilities

• Deliver compassionate, patient-centered hospice care and collaborate with an interdisciplinary team.
• Perform urgent assessments, manage symptoms and pain, and provide end-of-life support in real time.
• Support and guide patients and families during weekends and crises; ensure informed and reassured care throughout the hospice journey.
• Apply clinical judgment to coordinate care, respond to changing patient conditions, and maintain timely EMR documentation.
• Travel to patient homes within assigned territory.

• Work independently, but never alone

Weekend RN Qualifications

• Active Registered Nurse, RN license in the state you work

• Due to the field-based nature of this home hospice position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
