Job summary

Graham Healthcare Group is seeking a Weekend Registered Nurse (RN) to deliver end-of-life hospice care in patients’ homes around Brighton, MI. The role includes driving to patient locations, collaborating with an interdisciplinary team, and documenting care in the EMR system.

Qualifications

• Active RN license in the state you work (MI).
• Two years of RN experience; hospice experience preferred.
• Ability to drive to patient locations ~20% of time.
• Strong clinical judgment and ability to coordinate care.

Responsibilities

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life

• Respond to patient needs by performing urgent assessments, managing symptoms and pain, and providing compassionate end-of-life support in real time
• Support and guide patients and families during weekends, and crisis situations, ensuring they feel informed, reassured, and cared for throughout the hospice journey
• Apply strong clinical judgment and critical thinking to coordinate care needs, respond to changing patient conditions, and maintain accurate, timely, and compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

Weekend RN Qualifications

• Active Registered Nurse, RN license in the state you work
• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
