Job summary

JourneyCare Hospice in Chicago, IL seeks experienced Hospice RNs for evening shifts supporting patients and families with compassionate end‑of‑life care. You will assess physical, emotional, social, and spiritual needs, coordinating with the interdisciplinary team to plan and deliver comprehensive care.

Responsibilities include communicating with physicians and staff, providing death pronouncement support to families, and maintaining timely care plans and progress notes in the patient record.

Qualifications

• Graduate from an accredited registered nursing program.
• Current RN license to practice in the state.
• Minimum of two years of varied nursing experience.
• CPR certification required.
• Ability to cope with family emotional stress.

Responsibilities

• Obtain data on physical, psychological, social, and spiritual factors affecting patient and family needs.
• Coordinate with attending physician and interdisciplinary team for optimal care.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate clinical records with care plans and progress notes.

• Graduate from an accredited registered nursing program
• Possession of a current license to practice in the state as a Registered Nurse
• Minimum of two years of varied work experience as a professional nurse
• Demonstrated skill in nursing practices and clinical judgments
• Ability to cope with family emotional stress and tolerance of individual lifestyles
• Must possess current CPR Certification
• Valid driver’s license and proof of insurance is required

Schedule

Monday- Friday 4:30PM-12:30AM With Every 4th Weekend - 1 Summer and 1 Winter Holiday.

What You’ll Do:

• Obtain data on physical, psychological, social, and spiritual factors that may impact patient and family’s needs and initiate intervention by other members of the interdisciplinary team as appropriate
• Initiate communication with the attending physician, other interdisciplinary staff members and other agencies for the purpose of coordinating optimal care.
• Perform clinical pronouncement of death and provide support to family
• Maintain an accurate clinical record through the timely preparation of care plans and clinical and progress notes

Salary

$80,000 - $97,000 Annually - Offer Based on Years of Experience
