Job summary

JourneyCare Hospice in Chicago, IL is seeking Hospice RNs for an immediate opening. The role offers a generous time off package, 401(k) plus match, and a supportive team environment.

Responsibilities include coordinating interdisciplinary care, communicating with physicians, performing death pronouncements as needed, and maintaining comprehensive patient records. Schedule includes evenings with weekend rotation, serving North Chicago and Lake Bluff areas.

Qualifications

• Graduate from an accredited registered nursing program.
• Current license to practice as a Registered Nurse in IL.
• Minimum of two years of varied work experience as a professional nurse.
• Demonstrated nursing practices and clinical judgment.

Responsibilities

• Obtain data on physical, psychological, social, and spiritual factors affecting patient and family needs and coordinate care with the interdisciplinary team.
• Initiate communication with the attending physician and other staff for optimal care coordination.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate clinical records with timely care plans and progress notes.
