Job summary

Bristol Hospice Augusta, GA is seeking an Admissions RN to establish and maintain relationships with referrals and communities, guiding patients through the admission process. The Admissions RN is responsible for developing and increasing access to Bristol Hospice Services in the communities served.

This full-time daytime role requires GA RN license, travel about 20%, and a commitment to compassionate, patient-centered hospice care.

Qualifications

• Must have a current RN license in the state.
• Minimum of one (1) year of experience as a professional nurse within the last three (3) years or have a bachelor’s degree in nursing from a program accredited by the National League of Nursing.
• Must be able to travel 20% of the time, often without notice
• Must be flexible in work hours
• Must possess the willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

Responsibilities

• Relate information to internal stakeholders to promote hospice access.
• Establish and monitor community, customer, payer and patient perceptions of Bristol Hospice.
• Facilitate communication with agencies and referral sources.
• Maintain working knowledge of hospice educational resources for staff.
• Coordinate hospice referrals and admissions.
• Complete hospice consults, evaluations and admissions.
• Coordinate with ED and DPCS for planning and education programs.
• May participate in QAPI processes and IDT meetings.
• Provide patient/family education related to plan of care.
• Provide professional nursing care using the nursing process.
