Qualifications

• Must have current RN license in the state.
• Minimum 1 year of nursing experience within the last 3 years or a BSN from an NLN-accredited program.
• Must be able to travel ~20% of the time and flexible in work hours.
• Willingness to maintain knowledge of information systems and applicable software programs.

Responsibilities

• Establish and maintain relationships with customers, referral sources, and community education directors.
• Coordinate admissions and communications with hospice agencies and referral sources.
• Complete hospice consults/evaluations and admissions.
• Provide professional nursing care using all elements of the nursing process and assess patient/family needs.
