Job summary

Bristol Hospice located in State College, PA is seeking an Admissions RN to establish relationships with community and referral sources. The role focuses on expanding access to hospice services and coordinating admissions with hospital and facility partners.

The nurse will educate the community about hospice care, promote Bristol Hospice as a high‑quality provider, and support a timely admission process while traveling about 20% of the time.

Qualifications

• Must hold a current RN license in the state.
• Minimum of one year of professional nursing experience within the last three years, or a BSN from an NLN-accredited program.
• Willingness to travel 20% of the time, often with little notice.
• Flexibility in work hours and adaptability to schedules.
• Desire to maintain knowledge of information systems and applicable software.

Responsibilities

• Relate information to DPCS, ED and Community Education Director to promote hospice access.
• Assess and meet ongoing educational needs of the community related to hospice care.
• Promote Bristol Hospice as a high‑quality provider to the community, patients and payers.
• Facilitate communication with referral sources to ensure continuum of care.
• Coordinate and communicate with hospice leadership to support timely admissions.
• Complete hospice consults, evaluations and admissions as part of the intake process.
