Job summary

Bristol Hospice is seeking an Admissions RN to establish and maintain relationships with customers and referral sources across our Northern LA community, including Glendale, CA. The role focuses on promoting hospice access, coordinating admissions, and ensuring timely, compassionate care.

Requires CA RN license, travel about 20%, and ability to work in a fast-paced environment. The Admissions RN will develop community education initiatives, monitor perceptions of Bristol Hospice, and collaborate

Qualifications

• Relate information to DPCS, ED and Community Education Director of hospice access initiatives
• Assess and meet educational needs of the community related to hospice care
• Establish and monitor perceptions of Bristol Hospice as a high-quality provider

Responsibilities

• Coordinate and complete hospice consults/evaluations and admissions
• Facilitate communication with referral sources to promote care coordination
• Provide patient/family education related to the plan of care
• Work with intake to liaise between facility, physicians, hospitals and hospice programs
• Participate in IDT meetings to determine patient/family plan of care
