Qualifications
- Must hold a current RN license in Georgia.
- Minimum of one year nursing experience or a BSN from NLN-accredited program.
- Ability to travel as part of role, as needed for community outreach.
- Strong communication and interpersonal skills for education and referrals.
Responsibilities
- Establish and maintain relationships with customers and referral sources.
- Develop access to Bristol Hospice services within the community.
- Coordinate admissions and determine hospice care needs.
- Communicate plan of care with agencies, physicians, and hospitals.
- Provide professional nursing care using the nursing process.
- Participate in IDT meetings and quality improvement activities.
- Educate communities about hospice benefits and services.
- Support timely admissions and care coordination.
