Job summary

Bristol Hospice - Redding, CA is seeking an Admissions RN to establish and maintain relationships with customers and referral sources while improving access to hospice services. The Admissions RN is responsible for developing and increasing access to Bristol Hospice Services in the communities served, coordinating admissions and care plans with ED, DPCS, and community partners.

This full-time, day-shift role requires an active RN license, travel up to 20%, and a compassionate approach aligned

Qualifications

• Must have a current RN license in the state.
• Minimum 1 year of nursing experience within the last 3 years or a bachelor’s degree in nursing from a program accredited by the National League of Nursing
• Must travel 20% of the time and be flexible with work hours.
• Must possess the willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

Responsibilities

• Promote hospice access with community education and outreach.
• Assess and meet community education needs related to hospice.
• Manage perceptions of Bristol Hospice as a high-quality provider.
• Coordinate with referral sources to promote care continuum.
• Complete hospice consults, admissions, and assessments.
