Job summary

Affinity Care of Ohio seeks a Hospice RN Inpatient Admission Coordinator to serve as the primary link between referral sources, patients, and families. The role coordinates admissions, guides families through hospice services, and collaborates with the clinical team to ensure compassionate, timely transitions.

The ideal candidate has hospice/palliative care experience, strong communication skills, and a solid understanding of Medicare hospice regulations.

Qualifications

• Experience in hospice, palliative care, home health, or related healthcare field.
• Strong understanding of Medicare hospice regulations and eligibility criteria.
• Excellent communication, interpersonal, and presentation skills.
• Valid driver’s license and reliable transportation.
• RN license; sales or community outreach experience preferred.

Responsibilities

• Build and maintain relationships with hospitals, SNFs, physicians, social workers, and referral partners.
• Educate healthcare professionals about hospice philosophy, services, eligibility criteria, and Affinity Care of Ohio’s programs.
• Respond promptly to referrals and inquiries, ensuring timely follow-up and exceptional customer service.
• Assess hospice eligibility by reviewing clinical information and consulting with the hospice medical director.
• Coordinate and complete admission paperwork, consents, and required documentation in compliance with Medicare, Medicaid, and state regulations.
• Explain hospice services in person or virtual meetings; ensure smooth transitions by coordinating with the clinical team.
• Maintain accurate, timely documentation in the EMR system; participate in team meetings and improvement initiatives.
