Job summary

Reserve Care in Ohio seeks a Clinical Admissions Liaison-Registered Nurse to assess and admit patients to hospice services in home or facility settings. You will collaborate with the Referral Department to promote compassionate care and coordinate admissions efficiently.

The role requires an Ohio nursing license (BSN and/or CHPN preferred), at least one year of nursing experience in intake, home care, hospice or oncology, and a valid driver’s license.

Qualifications

• Graduate of an accredited nursing program: BSN and/or CHPN.
• Ohio nursing license required; valid driver's license with insured vehicle.
• Minimum of one year of nursing experience in intake, home care, hospice or oncology.

Responsibilities

• Provide information assessment and admission services to referred patients in home, facility, or health care settings.
• Communicate with Referral Department and physicians to ensure referrals are appropriate and documented.
• Maintain electronic medical records and coordinate with the IDT to ensure timely care.
• Deliver nursing care, education and emotional support to patients and families.
• Adhere to agency timelines and regulatory requirements for documentation.
