Job summary

Ohio's Hospice is seeking a Part-Time Admission Care Liaison (RN) to support patients and families as they transition to hospice care. You will conduct comprehensive assessments, educate families about services, and coordinate with hospitals, long-term care facilities, and home settings to ensure smooth admissions.

This role requires an Ohio RN license, CPR, strong communication, and ability to travel to patient sites.

Qualifications

• Graduate of an accredited nursing school.
• Current RN license in Ohio with no board actions.
• Current CPR certification.
• Computer skills sufficient for accurate clinical documentation.
• Valid Ohio driver's license and acceptable driving history.
• Current automobile insurance meeting state minimum requirements.
• Ability to travel and safely drive in varying weather conditions.
• Ability to pass a background check and 10-panel drug screen.

Responsibilities

• Conduct comprehensive assessments of patients and families, addressing physical, emotional, psychosocial, and spiritual needs.
• Evaluate patients in hospitals, long-term care facilities, and home settings.
• Educate patients and families about hospice services, eligibility, and available support.
• Help patients and families make informed decisions about hospice care.
• Collaborate with the interdisciplinary team to develop and initiate individualized plans of care.
• Communicate clinical findings and patient status to hospice physicians to support eligibility determination and certification.
• Provide ongoing assessments, symptom management, and care coordination for patients in contracted facilities.
• Help coordinate transitions between hospitals, facilities, and home settings.
• Partner with physicians, hospital personnel, facility staff, and other care teams to support continuity of care and timely discharge planning.
• Assist with organizational and patient care initiatives across departments as needed.
• Participate in quality improvement activities focused on patient outcomes and service excellence.
• Mentor and orient newly hired staff while promoting teamwork and professional development.
