Job summary

Ohio’s Hospice, Inc. in Wooster, OH seeks a full-time Admissions RN to perform comprehensive patient/family assessments and coordinate care across hospital, long-term care, and home settings.

You will educate families on hospice services, support informed decisions, and collaborate with physicians to ensure seamless admissions. Qualifications

• Graduate of an accredited nursing school.
• Current RN license in the state of Ohio with no board actions.
• Two years of acute care nursing experience preferred.
• CHPN certification preferred; preparation and support available.
• Current CPR certification.
• Computer skills to accurately document services and patient care.
• Valid driver’s license and acceptable driving history.
• Ability to travel as needed in varying weather and road conditions.
• Ability to complete a background check and drug screening.

Responsibilities

• Conduct comprehensive patient and family assessments addressing needs in hospitals, long-term care facilities, and home settings.
• Educate patients and families regarding hospice services, eligibility, and resources.
• Collaborate with interdisciplinary teams to develop individualized plans of care.
• Communicate findings to hospice physicians to support eligibility and certification.
• Provide ongoing assessments, symptom management, and care coordination.
• Promote continuity of care and timely discharge planning with care teams.
• Assist across departments and support organizational operations and initiatives.
• Participate in quality improvement activities and staff mentorship.
