Job summary

OhioHealth at Home in partnership with Compassus is seeking a Hospice Admission Registered Nurse (RN) to join our care team in Mansfield, Ohio. This part-time role offers 20 hours per week across two 10-hour shifts, with weekdays Monday, Tuesday, Thursday, and Friday coverage.

You’ll enable patients to spend quality time with loved ones while coordinating initial hospice assessments and care planning. Requirements include an active RN license in Ohio, at least two years of nursing experience

Qualifications

• RN license in the state you work.
• Two years of RN experience, hospice experience preferred.
• Current driver’s license.
• Current CPR and BCLS certification.

Responsibilities

• Enable patients to spend quality time with their loved ones and doing the activities they enjoy.
• Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers.
• Ensure all patient’s initial needs are addressed.
• Collaborate with the Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information.
