Job summary

HopeHealth Hospice & Palliative Care is seeking an experienced Registered Nurse (RN) Case Manager to provide compassionate care to hospice patients in their homes across Northern Rhode Island. The RN will oversee clinical care, advocate for patients, and lead the care team with urgency and precision.

The role requires 1–3 years of clinical experience, RI (and MA) licensure, and strong assessment/communication skills. Work schedule is 40 hours weekly, with 8am-4:30pm hours and rotating weekends.

Qualifications

• Graduate of an accredited nursing program; RN required; BSN preferred.
• Licensed as a Registered Nurse in RI (Massachusetts preferred).
• Minimum 1–3 years of clinical experience; strong assessment and communication skills.

Responsibilities

• Conducts initial and ongoing patient assessments.
• Collaborates with physician to manage patient plan of care.
• Evaluates and updates the plan of care to meet patient needs.
• Provides ongoing training and education to patients and families.
• Initiates preventive and rehabilitative nursing procedures and referrals.
• Maintains accurate, timely clinical documentation.
• Coordinates services and communicates changes to the team.
• Determines scope and frequency of services based on acuity.
