HopeHealth, Inc. in Providence, RI is seeking an experienced Registered Nurse (RN) for its Access Center.

This full-time role coordinates referrals and admissions to ensure timely hospice services, acting as a liaison among referral sources, care providers, staff, patients and families. This 40-hour-per-week position requires 2-3 years hospice experience, strong nursing assessment and communication skills, and the ability to schedule visits and resolve scheduling issues while supporting patients

• 2-3 years hospice experience required
• Strong nursing assessment and communication skills

Responsibilities

• Schedules visits for referred patients within required timeframe.
• Provides follow up on pending patients and non-admissions; communicates updates to referral sources.
• Helps resolve referral and admissions scheduling issues through direct communication with patients, referral sources, and Manager of Access.
• Provides clinical follow up with referral sources on upcoming discharges from hospitals.
• Admissions duties as needed
