Job summary

HopeHealth Hospice & Palliative Care is seeking a Registered Nurse (RN) Case Manager to oversee patient care at home and in facilities across Massachusetts. The role emphasizes advocacy, coordination, and timely documentation, with a 40-hour work week (8am-4:30pm) and rotating weekends.

The RN will assess patients, develop and adjust care plans with physicians, and educate patients and families. MA/RI licensure is preferred; BSN is desirable, with 1–3 years of clinical experience.

Qualifications

• RN license required; MA/RI preferred per state licensure.
• BSN preferred; clinical nursing experience in home health or hospice.
• Strong assessment, care planning, and patient/family communication.

Responsibilities

• Assess home and nursing home patients and develop a plan of care.
• Collaborate with physicians to manage and adjust patient plans.
• Provide training and education to patients and families.
• Coordinate services and document patient conditions and needs.
• Ensure timely, accurate clinical documentation and communication.
