Job summary

AccentCare in Fall River, MA is hiring an After Hours Hospice Admission RN. This full-time role requires current RN licensure in the state, prior hospice/home health experience preferred, and willingness to participate in an on-call rotation.

Requires strong communication, organizational skills, and proficiency with EMR systems and MS Office. The position involves on-site work with lifting, transporting supplies, and coordinating care with the IDG.

Qualifications

• Current licensure as a Registered Nurse in the applicable state is required, with an associate degree preferred.
• Must demonstrate willingness to participate in on-call rotation and have reliable transportation.

Responsibilities

• Conduct comprehensive nursing assessments and develop individualized care plans with the Interdisciplinary Group.
• Maintain timely documentation and ensure plans reflect patient status.
• Provide education and emotional support to patients and families; participate in on-call rotations and care transitions.
• Coordinate all aspects of patient care as Care Manager or On-Call, including scheduling and supervision where applicable.
