Job summary

VIA Health Partners in North Carolina seeks a Registered Nurse to coordinate admission care for patients and families in home or community settings. You will work with the patient, family, physician, and the hospice interdisciplinary group to establish a tailored plan of care and goals at the point of admission.

The role requires an active NC/SC RN license, a minimum of 2 years in hospice/palliative care, CPR, and CHPN preferred. Pediatric experience is required for Kids Path Nurse positions.

Qualifications

• Diploma or Associate’s Degree in nursing required; Bachelor’s Degree preferred.
• Current, active RN license in NC/SC.
• Current CPR required; CHPN preferred.
• Minimum 2 years related experience in hospice/palliative care.
• Pediatric experience required for Kids Path Nurse role.

Responsibilities

• Assesses admission nursing needs of patient/caregiver and documents findings.
• Develops and updates the plan of care with patient, family, physician, and the interdisciplinary group.
• Coordinates with the Physician of the Day and external providers to ensure timely care.
• Provides education to patient and caregiver at admission and during visits.
• Maintains accurate, timely clinical documentation per guidelines.
• Ensures infection control practices and safe, cost-effective delivery of care.
