Job summary

AuthoraCare Collective in Greensboro, NC is seeking an RN Admissions to join our hospice team. This full-time role covers Monday–Friday 11:00am–8:00pm with one weekend per month and two holidays per year.

You will explain hospice services, assist with consent, and collaborate with the interdisciplinary team to develop patient-centered plans of care for adults and geriatric patients. Qualified candidates will be licensed in NC, have nursing degree, and bring compassionate patient care, strong

Qualifications

• Graduate of an accredited School of Nursing.
• Must be licensed as an RN in North Carolina.
• One year of experience in acute, hospice, home health, or long-term care.
• BSN preferred; CHPN preferred.
• Driver's license and own transportation required.

Responsibilities

• Explain hospice services to patients and families and assist with consent.
• Coordinate and develop plan of care with the patient, family, and IDG.
• Assess, plan, implement, document, and coordinate care for adults and geriatric patients.
• Perform nursing duties with emphasis on patient-centered outcomes.
