Affinity Hospice Management LLC is seeking an Admission RN to plan and direct hospice care in home and community settings. The role emphasizes comprehensive patient assessment, nursing care, and coordination with an interdisciplinary team.

Qualified candidates will provide education to patients and families, administer treatments, and support end-of-life care planning in a community-based environment. This position requires strong clinical skills and collaboration with the Clinical Director.

Responsibilities

• Completes initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care using all elements of the nursing process.
• Writes and initiates plan of care; reevaluates patient and family needs.
• Initiates and revises the plan of care as patient status changes.
• Uses health data to determine nursing diagnosis and goals.
• Develops a care plan with patient/family involvement and pallia tive actions.
• Administers medications and treatments as prescribed.
• Counsels patient and family in nursing and related needs.
• Provides health care instructions per assessment and plan.
• Assists with activities of daily living and supports self-sufficiency.
• Acts as Case Manager when assigned and coordinates patient care.
