You will work with patients, families, physicians and the hospice interdisciplinary group to anticipate needs and develop specific plan of care goals.

Qualifications

• Requires active RN license in NC/SC or eligible.
• CPR certification required; CHPN preferred.

Responsibilities

• Conduct admission nursing assessment to identify patient/caregiver needs.
• Develop plan of care addressing physical, emotional, spiritual needs.
• Coordinate with physicians, family, and interdisciplinary team.
• Ensure documentation and education provided at admission.
• Maintain professional relations with external healthcare providers.
